{"id":3438,"date":"2025-03-22T09:12:41","date_gmt":"2025-03-22T08:12:41","guid":{"rendered":"https:\/\/trajectoiresjeunestrans.fr\/non-categorise\/germany-austria-and-switzerland-commit-to-supporting-gender-affirmation-for-trans-minors\/"},"modified":"2026-01-25T15:46:43","modified_gmt":"2026-01-25T14:46:43","slug":"germany-austria-and-switzerland-commit-to-supporting-gender-affirmation-for-trans-minors","status":"publish","type":"post","link":"https:\/\/trajectoiresjeunestrans.fr\/en\/news\/germany-austria-and-switzerland-commit-to-supporting-gender-affirmation-for-trans-minors\/","title":{"rendered":"Germany, Austria and Switzerland commit to supporting gender affirmation for trans minors"},"content":{"rendered":"<div class=\"wpb-content-wrapper\"><p>[vc_row][vc_column][vc_column_text css=&#8221;&#8221;]After several years of work, the Association of Scientific Medical Societies (AWMF), an organisation bringing together numerous German-speaking professional societies, has published a scientific consensus text on gender-affirming care specific to trans adolescents, spanning 425 pages. This consensus involved very broad participation: 26 professional medical and psychotherapeutic organisations, as well as two organisations of people with lived experiences, contributed to building the consensus, which applies to Germany, Austria and Switzerland. <\/p>\n<p>From the outset, the professional societies highlight the delicate ethical context of producing definitive scientific evidence on care for trans adolescent populations, due to the absence of proven effectiveness of non-affirming approaches: \u201c<strong>As there is no proven effective alternative treatment for persistent gender incongruence with gender dysphoric distress that works without body-altering medical interventions<\/strong>, the feasibility of controlled clinical trials is also severely limited for ethical reasons. This means that the available evidence is mainly based on clinical observational studies.\u201d<\/p>\n<p>In the absence of randomised controlled trials, the clinical experience of professionals is even more necessary in developing guidelines. This approach, based on the experience of professionals working with trans youth and directly observing the effects of gender-affirming care prescribed to them, is fundamentally different from the approach followed by the Cass report (<span style=\"text-decoration: underline;\"><a href=\"https:\/\/trajectoiresjeunestrans.fr\/en\/news\/critical-evaluation-of-the-cass-report-methodological-flaws-and-unfounded-claims\/\" target=\"_blank\" rel=\"noopener\">previously discussed on our site<\/a><\/span>) \u2013 \u201cSome national health authorities&#8217; guidelines, such as the Cass Review for NHS England and Wales (Cass, 2024), recommend a comparatively more restrictive approach to adolescents&#8217; access to body-altering interventions on the grounds that the evidence base is uncertain. <strong>The involvement of clinical experts in the preparation of these recommendations is not assured or at least not transparent or questionable<\/strong>.\u201d <\/p>\n<p>On this matter, the AWMF supports its position by citing the statement of the German Federal Medical Association (2020):<br \/>\u201cEvidence-based medicine is the conscientious, explicit and judicious use of current best external scientific evidence to inform decisions in the medical care of individual patients. The practice of evidence-based medicine is the integration of individual clinical expertise with the best available external evidence from systematic research. Evidence-based medicine is based on three pillars: <strong>individual clinical experience, the values and wishes of the patient and the current state of research<\/strong>.\u201d  <\/p>\n<p>Regarding ethical debates on gender-affirming care, the AWMF societies recognise that it is necessary to consider \u201cOn the one hand, the irreversible consequences of a treatment decision in favour of hormone treatment\u201d, but also, \u201cOn the other hand, postponing such treatment due to the equally irreversible progression of the development of male or female physical characteristics <strong>can lead to an aggravation of gender dysphoric distress and thus to the worsening and chronification of gender dysphoria<\/strong>.\u201d The AWMF emphasises, echoing the German Ethics Council\u2019s requirement, that a \u201ccareful consideration of the benefits and risks of both the contemplated treatment and the decision <strong>not to undergo such treatment <\/strong> when making medical treatment decisions in adolescence.\u201d In this ethical balancing, &#8220;The principles of &#8220;non-maleficence&#8221;, &#8220;beneficence&#8221;, &#8220;autonomy&#8221; and &#8220;justice&#8221; should be observed equally and in a balanced manner. This means that experimental or outdated methods should not be used uncritically, but treatments should be used in a well-considered manner and with a careful risk-benefit assessment. A risk-benefit <strong>assessment of non-treatment<\/strong> should also be carried out.&#8221; with the latter potentially &#8220;can be associated with <strong>an increased risk of impaired long-term mental health<\/strong>.&#8221;  <\/p>\n<p>Referring to the construction of the expert consensus, the authors of the text recognise the possibility of dissent, especially on basic ethical and therapeutic positions, which are systematically documented. Most evidence-based recommendations, however, show strong consensus (&gt; 95 % approval). <\/p>\n<p>How are guideline recommendations justified when evidence is uncertain? The AWMF professional societies directly address this question and justify their methodology as follows: \u201cGuideline recommendations recognised by the medical community for which there is only uncertain or very uncertain evidence are widespread in medicine. This is particularly true for paediatric medicine. According to a recent meta-analysis, <strong>82% of all treatment recommendations in medical guidelines for off-label prescriptions in paediatric medicine are based on uncertain to very uncertain evidence<\/strong> (Meng et al., 2022). If one considers that <strong>in paediatric medicine up to 65% of all medications prescribed in the inpatient sector and up to 31% of all medications prescribed in the outpatient sector are only available in off-label use<\/strong> (Kimland &amp; Odlind, 2012), one can estimate the scale of established guideline recommendations for children and adolescents that are based on uncertain to very uncertain evidence only. In such cases, until research gaps are closed by further studies, consensus-based guideline recommendations\u2014aligned with recognized best clinical practice\u2014are generally justified in pediatric medicine if, alongside positive clinical experience, the following conditions are fully or partially met (Meng et al., 2022):\u201d     <\/p>\n<ul>\n<li>There is sufficient evidence in favour of treatment for adults,<\/li>\n<li>the medical mechanism of action of a particular drug is considered to be fully known, and sufficient patient safety in children and adolescents has been proven for another area of application,<\/li>\n<li>there is no other justifiable and tested alternative treatment available,<\/li>\n<li>non-treatment is ethically unacceptable,<\/li>\n<\/ul>\n<p>The AWMF says: &#8220;The consensus-based recommendations in this guideline on the use of body-altering medical interventions in adolescents with persistent gender incongruence are also based on these generally recognised and common considerations and procedures in paediatric medicine.&#8221;<\/p>\n<p><strong>The recommendations:<\/strong><\/p>\n<p><strong>On social transitions in childhood<\/strong><\/p>\n<p>With regard to recommendations on social transitions before puberty, the experts agree that these fall under the domain of law, not medicine :<br \/>\u201cWhen counselling children with gender incongruence or gender dysphoria who are considering a social transition before the onset of puberty and their guardians and possibly other caregivers, professionals <strong>should respect the child&#8217;s right to free development of their personality<\/strong>.\u201d<\/p>\n<p>This does not mean, however, that health professionals cannot be involved, particularly in supporting parents in gender exploration and affirmation: \u201cProfessionals should try to sensitise the legal guardians to an attitude that enables the child to explore and develop their gender identity and social gender role in a self-determined way.\u201d<br \/>Health professionals are also expected to act in response to the marginalisation that a child may experience:<br \/>\u201cProfessionals should offer their support to protect the child and\/or their caregivers from stigmatisation and discrimination, regardless of the individual decision and life path of those affected..\u201d<\/p>\n<p><strong>On puberty blockers in adolescence<\/strong><\/p>\n<p>With regard to the initiation of puberty blockers for trans adolescents, the recommendations of the German, Austrian and Swiss professional societies align with those of other international guidelines (including WPATH) :<br \/>\u201cThe recommendation for puberty blockers in adolescents with gender incongruence or gender dysphoria should be made in interdisciplinary collaboration and after careful consideration of potential risks and after detailed information for the adolescents and their families. A prerequisite for this recommendation <strong>is a careful diagnostic assessment and clarification appropriate to the urgency and complexity of the individual situation<\/strong>. It also requires a differentiated and individual risk assessment after detailed information has been provided to patients and their families by a mental health specialist experienced in the diagnosis and treatment of gender dysphoria in children and adolescents. With regard to the risks, it is essential to provide information about the uncertain state of evidence as it stands today, with a lack of long-term studies. The somatic part of the indication should be carried out by an experienced paediatric endocrinologist with regard to its prerequisites (stage of pubertal development, absence of somatic contraindications, etc.)\u201d More specifically, &#8220;A prerequisite for the recommendation of puberty suppression should be the presence of stable\/persistent gender incongruence with gender dysphoric distress that has arisen or intensified after the onset of puberty.&#8221;   <\/p>\n<p>Here again, the expert societies emphasise the ethical balancing of treatment and non-treatment :<br \/>\u201cThe justification for the recommendation of pubertal blockade should contain an ethically reflected risk-benefit assessment related to the individual case, <strong>both of the intended treatment and of not initiating this treatment or waiting until a later date<\/strong>.\u201d<\/p>\n<p>Thus, in situations of urgency, care teams may need to adopt a flexible approach: &#8220;If, in individual cases, time pressure arises as a result of progressive pubertal maturation, in which irreversible physical changes (e.g. male voice change) could be expected to cause damage to health as a result of longer waiting times, access to child and adolescent mental health assessment and medical treatment options should be granted <strong>as soon as possible<\/strong>.&#8221;<\/p>\n<p>It is important to emphasise that, according to the German-language consensus, having undergone a social transition is not a prerequisite for access to puberty blockers, making individuals who are still exploring or questioning their gender eligible for this treatment: &#8220;An already initiated or completed social role change should not be considered a necessary criterion when determining the recommendation for a puberty suppression.&#8221; <\/p>\n<p><strong>On hormone treatments in adolescence<\/strong><\/p>\n<p>With regard to hormone treatments (estradiol, testosterone), the professional societies make their prescription subject to the same criteria as puberty blockers :<br \/>\u201cThe recommendation for gender-affirming hormone treatment in adolescents with gender incongruence or gender dysphoria should be determined via multi-disciplinary cooperation. The prerequisite for a recommendation is a careful diagnostic assessment and <strong>clarification appropriate to the urgency and complexity of the individual situation<\/strong> by a mental health specialist experienced in the diagnosis and treatment of gender dysphoria in adolescents. The medical part of the recommendation should be carried out by an endocrinological specialist experienced in the treatment of adolescents, taking into account their prerequisites (pubertal stage of maturity, absence of somatic contraindications, etc.).\u201d More specifically, in addition to the individual\u2019s expressed need for this treatment, \u201cA prerequisite for the recommendation of gender-affirming hormone treatment should be the presence of stable\/persistent gender incongruence with gender dysphoric distress present after the onset of puberty with several years of transgender perception and the associated desire for the development of the gender-specific physical changes to be expected as a result of hormone treatment.\u201d   <\/p>\n<p>As with puberty blockers, ethical reflection must include the risks associated with non-intervention:<br \/>\u201cThe justification for the recommendation of gender-affirming hormone treatment should contain an ethically reflected risk-benefit assessment based on the individual case, both of the planned treatment and of not initiating this treatment or waiting until a later date.\u201d<\/p>\n<p>It is important to emphasise that sexual orientation, whatever it may be, must not be an inclusion or exclusion criterion for clinicians:<br \/>\u201cThe recommendation for gender-affirming hormone treatment should be made regardless of the polarity or binary nature of gender identity and regardless of the patient&#8217;s sexual orientation.\u201d<\/p>\n<p>According to the authors, \u201ca social trial of the desired gender role should take place\u201d is a condition for access to hormone treatments, insofar as \u201cprovided this is compatible with the protection against discrimination\u201d, in which case \u201cpsychotherapeutic support for the transition process should be offered.\u201d<\/p>\n<p>Clinicians involved in prescribing sex hormones must also be able to offer fertility preservation consultations, in light of the uncertainties of hormone treatments regarding future fertility.<\/p>\n<p><strong>On surgeries in adolescence<\/strong><\/p>\n<p>With regard to chest surgery (mastectomy \/ chest reconstruction) for transmasculine minors, the AWMF professional societies make access subject to the same general criteria as puberty blockers or hormone treatments. In addition, \u201cThe prerequisite for the recommendation of a gender-affirming mastectomy or surgical breast reduction should be the presence of a gender incongruence that has been stable\/persistent for several years with gender dysphoric distress combined with a clear desire for a change in the organ or characteristic to be operated on. The careful diagnostic assessment of the stability\/persistence of gender incongruence and the desire for treatment should be carried out in collaboration between themental health specialist experienced in the diagnosis and treatment of gender dysphoria in adolescence and the patient and their carers on the basis of careful exploration of the psychological findings and life history.&#8221; <\/p>\n<p>Once again, ethical reflection must weigh \u201crisk-benefit assessment based on the individual case,\u201d \u201cboth of the planned intervention and of not performing this intervention or waiting until a later date.\u201d<\/p>\n<p>As with hormone treatments, \u201csocial testing of the desired gender role should take place, provided this is compatible with protection against discrimination,\u201d in which case \u201cpsychotherapeutic support for the transition process should be offered.\u201d<\/p>\n<p>Furthermore, where justified for the individual, chest surgery \u201cmay be considered for adolescents with gender incongruence or gender dysphoria without prior gender-affirming hormone treatment\u201d<\/p>\n<p><strong>International comparisons of recommendations<\/strong><\/p>\n<p>In an appendix to the recommendations, the AWMF professional societies graded the dozen or so different existing national or international guidelines on care specific to trans minors.[\/vc_column_text]<div class=\"grve-empty-space grve-height-1x\"><\/div>[vc_column_text css=&#8221;.vc_custom_1742631288211{padding-bottom: 12px !important;}&#8221; text_style=&#8221;small-text&#8221;]<strong>Table of systematic evaluations of the methodological quality of all guidelines reviewed according to the DELBI protocol:<\/strong>[\/vc_column_text]<div class=\"grve-element grve-image grve-align-center grve-light-gallery\">\r\n\t<div class=\"grve-image-item\">\r\n\t\t\r\n\r\n<div class=\"grve-image-wrapper grve-shadow-medium grve-no-effect\">\r\n\t\t<a class=\"grve-item-url\" href=\"https:\/\/trajectoiresjeunestrans.fr\/wp-content\/uploads\/2026\/01\/awmf.png\"><span class=\"screen-reader-text\">More<\/span><\/a>\t\t<div class=\"grve-thumbnail-wrapper\"  style=\"width: 800px;\"><div class=\"grve-thumbnail\"  style=\"padding-top: 31.25%;\"><img loading=\"lazy\" decoding=\"async\" width=\"800\" height=\"250\" src=\"https:\/\/trajectoiresjeunestrans.fr\/wp-content\/uploads\/2025\/03\/evaluations-systematiques-1x.png\" class=\"attachment-full size-full\" alt=\"\" srcset=\"https:\/\/trajectoiresjeunestrans.fr\/wp-content\/uploads\/2025\/03\/evaluations-systematiques-1x.png 1x,https:\/\/trajectoiresjeunestrans.fr\/wp-content\/uploads\/2026\/01\/awmf.png 2x\" data-column-space=\"auto\" data-lazyload=\"\" data-grve-filter=\"yes\" \/><\/div><\/div>\t<\/div>\r\n\t<\/div>\r\n<\/div>\r\n<div class=\"grve-empty-space grve-height-1x\"><\/div>[vc_column_text css=&#8221;&#8221;]These scores, ranging from 0 (lowest quality) to 1 (highest quality) across seven different domains (see Table), illustrate the general difficulty of developing recommendations on this topic, given the generally low scores reported for the guidelines assessed using the DELBI protocol. The guidelines receiving the highest average score across the six domains &#8211; excluding Domain 7, \u201capplicability in Germany\u201d &#8211; are those of WPATH. On this matter, the AWMF notes that \u201cno deviating recommendations recognisable\u201d exist between the WPATH guidelines and those of the AWMF.  <\/p>\n<p><strong>Comparison with the Cass Review<\/strong><\/p>\n<p>The AWMF extensively discussed the English NHS recommendations (the Cass Review), which are often invoked to restrict or prohibit care for trans minors, as is the case, <span style=\"text-decoration: underline;\"><a href=\"https:\/\/www.erininthemorning.com\/p\/new-german-swiss-and-austria-guidelines\" target=\"_blank\" rel=\"noopener\">for example, in the United States<\/a><\/span>.<\/p>\n<p>The AWMF contacted the NHS, which confirmed that Hilary Cass <strong>was the sole author of the report<\/strong>. The AWMF adds: \u201cIt is not documented which individuals were involved in the development of the review and in what way. The appointment of Hillary Cass as chair of the review was made by NHS England (Cass, 2024). Hillary Cass is a renowned paediatrician <strong>with no prior experience in the treatment of or research in gender dysphoria in children and adolescents<\/strong>. Apparently, no professional associations or scientific societies were consulted or involved in the development. A &#8221;Assurance Group&#8217; was appointed, but was explicitly not involved in the development of the Cass Review\u2019s recommendations. There are reports that an &#8216;Advisory Board&#8217; was also established. However, the members or potential influence of this board are not documented (Ruuska et al., 2024; Cass, 2024). <strong>Therefore, no assessment can be made reagarding the expertise of the contributors\/authors.<\/strong>\u201d       <\/p>\n<p>Probably linked to the fact that the Cass Review has a single declared author, \u201cNo formal consensus procedure (e.g. DELPHI) was specified and <strong>it remains unclear which individuals were involved<\/strong> in the wording of recommendations and the wording of the rest of the report. The effects and side effects of physical medicine interventions were discussed within the text;these <strong>were not systematically weighed against one another<\/strong>. In contrast, the effects and side effects of psychosocial interventions or possible negative consequences of the reviews recommendations (e.g. possible underuse) <strong>are scarely adressed<\/strong>.\u201d  <\/p>\n<p>While the Cass Review proposes conditioning access to care &#8211; such as puberty blockers &#8211; on clinical research, the AWMF highlights the limits of this proposal: \u201cAn extensive research programme is proposed. This does not explicitly refer to an evaluation of the recommendations of the Cass Review. A barrier analysis was not conducted (although training is identified as a relevant factor) and financial implications are barely discussed.\u201d<\/p>\n<p>The AWMF also describes a lack of transparency regarding potential conflicts of interest: \u201cNo documentation regarding the funding of the review is provided. <strong>The possible conflicts of interest of those involved in the development of the Cass Review are not disclosed. Individuals involved, aside from Hillary Cass, are not identified.<\/strong>\u201d <\/p>\n<p>The divergences between the AWMF guidelines and those of Hilary Cass are numerous, with the most notable relating to care provision and its organisation. According to Cass, \u201cPuberty blocking hormones should only be prescribed as part of a clinical trail. (Recommendation 6)\u201d; \u201cGender affirming hormones should only be prescribed \u201dwith extreme caution\u201c. There should be a clear clinical rationale for not delaying the prescription until the age of 18. Implicit: No prescription possible before the age of 16. (Recommendation 8).\u201d Furthermore, the AWMF notes that Cass proposes that \u201cEach case of medical treatment should be decided in advance at national level by a multidisciplinary team. (Recommendation 9).\u201d      <\/p>\n<p>Rating the methodological rigour of the Cass Review at 0.38 (on a scale from 0 to 1), the AWMF explains: \u201cOverall, the methodological rigour of development can be <strong>categorised as low to medium<\/strong>. As before, the areas diverge greatly. One positive aspect is that a high-quality literature search and selection process was conducted . The only issue is that different exclusion criteria regarding the methodological quality of the original studies were applied for medical interventions compared to psychosocial interventions. Consequently, the strengths and weaknesses of the evidence in the qualitative synthesis were not analysed in a consistent manner. The same applies to the weighing of effects and harms of the interventions. In the review of psychosocial interventions, the statement was made that there was \u201dno indication of adverse or negative effects\u201c in the reviewed original studies (Heathcote et al., 2024, p. 13). <strong>However, this is incorrect: Negative effects were described in some included studies<\/strong> (e.g. Bluth et al., 2021; Lucassen et al., 2020). A negative aspect is that no consensus finding procedure is specified. <strong>As no authors were named in addition to the chairperson, it appears that all recommendations were ultimately determined by the chairperson alone and not by a group representative of the field in a structured process and on the basis of the evidence obtained<\/strong>. Furthermore, no external review was conducted prior to publication.&#8221;       <\/p>\n<p>The AWMF strongly criticises certain recommendations of the Cass Review: \u201cThe rationale for recommending psychotherapy and other psychosocial interventions for GD is not clearly defined. Psychotherapy is recommended for co-occurring conditions, for which there is already an indication due to the co-occurring disorder itself. However, it is also recommended for the &#8216;management of gender-related distress&#8217;. <strong>None of the studies included in the review in question were able to show a reduction in gender dysphoria<\/strong> as a result of psychotherapy. Also, as shown above, the outcomes in the original studies for psychosocial interventions were inconsistent (in one case even negative compared to cis youth), there was a decline in positive effects after a follow-up period and possible adverse effects were not discussed.\u201d <\/p>\n<p>Furthermore, the AWMF notes: \u201cIt has already been discussed that the recommendation to exercise \u201dextreme caution\u201c when prescribing sex reassignment hormones is ambiguous. In contrast to puberty blockade, the outcomes of gender affirming hormones in minors in the reviewed original studies (Taylor et al., 2024d) <strong>point in the same direction towards an improvement<\/strong>, even if the study quality was almost consistently low. <strong>It would have been necessary to discuss not only the positive and negative effects of this intervention, but also in comparison with treatment alternatives<\/strong>.\u201d <\/p>\n<p><strong>Comparison with the statement of the National Academy of Medicine (France)<\/strong><\/p>\n<p>The National Academy of Medicine <span style=\"text-decoration: underline;\"><a href=\"https:\/\/www.academie-medecine.fr\/la-medecine-face-a-la-transidentite-de-genre-chez-les-enfants-et-les-adolescents\/\" target=\"_blank\" rel=\"noopener\">published a statement in 2022<\/a><\/span> which, to date and pending the publication of good practice guidelines by the Haute Autorit\u00e9 de Sant\u00e9 (HAS), remains the only French text issued by a medical authority that has taken a position on care for trans minors. The Academy\u2019s text differs substantially from that of the AWMF, notably regarding a recommendation of \u201cCaution regarding social media in children with gender dysphoria, assuming that the influence of social media in the sense of influencing young people is involved in the increasing prevalence of GD in adolescence.\u201d <\/p>\n<p>The AWMF assessed this text very harshly: &#8220;<strong>No methodology is reported<\/strong>. A voting result the Acad\u00e9mie is given. Authors are not named. <strong>There is no recognisable specific expertise<\/strong> for the healthcare of children with GD in the decision-making group. <strong>No recognisable experts were consulted<\/strong>.&#8221;  <\/p>\n<p>&#8220;<strong>The methodological rigour of development must also be rated as very low<\/strong>. <strong>There is no recognisable systematic search for, selection or evaluation of evidence<\/strong>. Also, no structured consensus process beyond a simple vote is reported. No review or consultation outside the Acad\u00e9mie took place and no validity period or updating procedure is specified.&#8221;<\/p>\n<p>This very negative evaluation of the methodology used by the National Academy of Medicine &#8211; the lowest-rated of the 12 guidelines assessed &#8211; stands in stark contrast to the considerable media coverage its statement received, both in France and internationally. It should encourage public decision-makers to rely on genuinely evidence-based medicine, which includes, as the AWMF does, drawing on the clinical expertise of frontline professionals as well as on the values and preferences of the people directly concerned.<\/p>\n<p>In France, the good practice guidelines of the Haute Autorit\u00e9 de Sant\u00e9 rely heavily on the state of scientific knowledge, clinical expertise, and the values and preferences of the people concerned, and are tasked with ruling on care for trans minors aged 16 and over. In a notable contrast to the Cass Review or the statement of the National Academy of Medicine, <span style=\"text-decoration: underline;\"><a href=\"https:\/\/reseausantetrans.fr\/2025\/01\/14\/tribune-jeunes-trans-la-haute-autorite-de-sante-se-doit-de-proteger-les-mineurs-des-attaques-transphobes\/\" target=\"_blank\" rel=\"noopener\">a high level of transparency<\/a><\/span> has been observed, both regarding the composition of the working group and the declarations of interests of its members. A consensus-seeking procedure following a systematic review of the literature, broadly similar to that used by the AWMF, has been implemented. These recommendations are expected to be made publicly available by mid-2025.   <\/p>\n<p><a href=\"https:\/\/www.awmf.org\/service\/awmf-aktuell\/geschlechtsinkongruenz-und-geschlechtsdysphorie-im-kindes-und-jugendalter-diagnostik-und-behandlung\" target=\"_blank\" rel=\"noopener\"><span
