According to the DSM-5 of the American Psychiatric Association, it involves clinically significant distress in addition to being trans. Dysphoria is often - incorrectly - used as a shorthand to summarize trans identity. Although such distress is real, it is important to recall that it is neither systematic nor mandatory in order to justify access to medical care, and it is far from encompassing what trans identity is.
